How do you set up a letter properly using MLA format? - letter for disonnection of telephone line

I must write a letter to a teacher for AP English and must apply the letter, there is MLA format. Thanks in advance! (:

Interesting - really the style member of the research, essays, etc., and not for correspondence. What is your teacher May on the search for a form letter in this direction:

General Guidelines

Type your paper into the computer and printed on standard white 8.5 x 11 inches.
